Who is on the provider side
The institutional counterparty is the Oklahoma Department of Corrections. The agreement is signed by the individual health-care provider treating the inmate; the form does not designate one named physician for all patients.
Terms that matter most
- The patient agrees to participate in other treatment requested by the provider.
- Selling or sharing medication can stop treatment.
- The form prohibits listed illicit substances and says treatment may stop after use.
- Random urine testing may be required at provider discretion.
- Specified noncompliance may result in cessation of controlled-drug prescribing.
- If prescribing is stopped for noncompliance, the form states that medication will be tapered over several days as necessary to avoid withdrawal.
Authority and scope
ODOC policy OP-140701 identifies DOC 140701E as an appropriate consent form to be completed and filed in the inmate’s electronic health record. That establishes an institutional correctional requirement; it does not convert every clause into a generally applicable Oklahoma outpatient requirement.
