Every consequential physician-, practice-, or contract-specific statement must have a preserved source behind it. Unknown facts remain unknown. Practice responses are attributable. Legal interpretation is separated from document fact.

Source hierarchy

  1. Controlling primary authority: Oklahoma statutes, enacted session laws, administrative rules, court records, and official agency materials.
  2. Professional record: Oklahoma licensing-board records and public disciplinary orders.
  3. Federal identity/business data: NPPES/NPI records, used as supplementary professional data and not as proof of licensure.
  4. Practice-origin evidence: current practice website, practice-supplied agreement, written correction, or attributable response.
  5. Other lawfully obtained agreements: patient-supplied or publicly archived forms, with current-use status stated accurately.

Contract classifications

  • Required by Oklahoma law: the controlling authority requires the term in the circumstances documented.
  • Authorized or contemplated: law allows or anticipates the subject, but does not require this exact implementation.
  • Program-specific: tied to Medicaid, workers’ compensation, payer rules, a profession, or another limited program.
  • Practice-selected: the office chose the term and no uniform statewide requirement has been established for it.
  • Broader than authority: the practice’s measurable restriction goes beyond what the cited statewide authority actually says.
  • Unresolved / legal review: the evidence is incomplete, conflicting, or would require a legal conclusion not established by the document alone.

Practice-verification statuses

  • Proactively Verified: the practice supplied the current document or verification without repeated follow-up.
  • Verified After Request: the practice supplied or confirmed the current record after a documented request.
  • Partially Verified: the practice responded but material requested information remains unresolved.
  • Declined Verification: the practice expressly declined to supply or verify requested information.
  • No Response: no response was received after the documented verification process.
  • Not Yet Contacted: Seeds of Vice has not yet made the verification request.
  • Version Disputed: the practice disputes the version or provenance and the conflict remains under review.

No-response and declined statuses describe documented communication outcomes only. They do not state or imply motive.

Financially connected treatment requirements

When a contract conditions continued opioid therapy on another service, Seeds of Vice records the literal requirement, consequence for refusal, who provides and bills for the service, whether outside providers are allowed, and any reliably established ownership or financial relationship. The public record does not label the arrangement illegal merely from those facts.

Potential legal significance is separately flagged for qualified legal review. The evidence record preserves the contract language and underlying ownership/business sources so a reviewer can reproduce the conclusion.

Correction policy

A physician or practice may submit a correction at any time. Seeds of Vice checks the correction against available evidence, updates supported factual errors, preserves material prior versions where appropriate, and records the reason for the change. Unsupported requests to remove accurate historical evidence are not treated as corrections.
