Evidence status: CURRENTLY LISTED AND LINKED BY THE CLINIC. The Broadway Clinic / FedCare’s live forms system lists a Controlled Substance Agreement for patients receiving pain medication and links directly to the two-page public agreement reviewed here.

Quick read

The two-page form combines medication rules with treatment-participation requirements. It addresses refills, prescriptions from other providers, urine testing, physical-therapy attendance, prescription-monitoring review, and termination.

Who is on the provider side

The public agreement names Steve Randall, MD; Kory Reed, PA-C, MHS; and Richard Beevers, APRN-CNP. Their names are evidence of who was identified on this version of the agreement; this page does not assume each remains in the same present role without separate current verification.

Terms that matter most

  • Medication changes and refills are handled through visits and office procedures stated in the form.
  • Lost or stolen medication does not automatically justify an early refill.
  • Outside pain prescriptions can affect whether the clinic continues prescribing.
  • Urine testing and prescription-monitoring review are used as compliance tools.
  • Repeated failure to attend ordered physical therapy can affect medication refills or continued treatment.
  • The agreement states several grounds for termination.

Law versus clinic policy

The PMP has an independent statutory framework. Oklahoma law also allows agreements to address refills, other prescribers, testing, and termination. The Broadway Clinic’s specific physical-therapy condition, refill implementation, and termination triggers are clinic-level provisions unless separately required by another applicable program.

Evidence boundary: The document is evidence of written clinic terms. Names printed on the form establish document provenance, not necessarily each person’s current employment status.
