  • The WVHA implementation guide states that WVHA developed the template for member hospitals based on the statutory narcotics-contract requirements.
  • The guide identifies the state-required contract subjects: prescribing practitioner, single pharmacy, 72-hour emergency notification, and breach consequences. Current statute additionally contains the approved-additional-physician provision.

What is not yet verified

The exact current Addendum A text, layout, signature fields, and any language beyond the statutory minimum have not been captured. Seeds of Vice therefore does not reconstruct the template from memory or assume that the guide’s statutory summary is identical to every line of the form.

Why this is a high-value document target

This template was specifically developed to operationalize West Virginia’s narcotics-contract law across member hospitals. A clean current copy would make it possible to distinguish the state’s minimum wording from implementation language chosen by WVHA.
