Help build Minnesota’s Contract-for-Care record
Minnesota has two separate government agreement systems: a statutory patient-provider agreement for intractable-pain treatment and a detailed workers’ compensation written opioid treatment contract for long-term opioid therapy. Seeds of Vice is collecting actual Minnesota agreements so those authorities can be distinguished from clinic-created terms.
What is most useful
- A blank patient-provider agreement used for treatment under Minn. Stat. §152.125.
- A Minnesota workers’ compensation written opioid treatment contract used under Minn. R. 5221.6110.
- The complete packet if the agreement came with drug-testing, refill, lost-medication, PMP, pharmacy, cannabis, taper, dismissal, conduct, or functional-goal policies.
- The revision date, footer, clinic name, prescriber name, employer/insurer information, and document number.
- Older versions of the same agreement so statutory, rule, and institutional changes can be tracked.
- Written notices showing how alleged nonadherence, diversion, abnormal testing, tapering, discontinuation, or referral was handled.
Protect patient and claim information
A blank form is best. On a completed copy, permanently redact the patient’s name, birth date, address, contact information, signatures, claim number, employer, insurer identifiers, medical-record numbers, individualized diagnoses and prescriptions, test results, barcodes or QR codes, and other personal information before submission.
How the document is classified
Each document is first classified as an intractable-pain statutory agreement, workers’ compensation treatment contract, private clinic agreement, or payer/institutional policy. Agreement clauses are then compared with the correct controlling authority rather than blended across systems.
Submit
Use the national Seeds of Vice submission channel and identify the document as a Minnesota Contract for Care and specify whether it is an intractable-pain agreement or workers’ compensation opioid contract.
