Help build Michigan’s Contract-for-Care record

Michigan uses different opioid-related forms for different purposes. Seeds of Vice is collecting the actual documents so Medicaid coverage conditions, workers’ compensation rules, statewide opioid education, and private clinic additions can be separated instead of blended together.

What is most useful

  • A blank Michigan pain-management, opioid, controlled-substance, or medication agreement.
  • A Pain Medication Agreement used for Michigan Medicaid high-MME prior authorization.
  • A Michigan workers’ compensation opioid treatment agreement.
  • A provider-modified version of the official WDCA sample agreement.
  • The complete packet if the agreement came with urine testing, pill counts, refill rules, one-pharmacy terms, cannabis/alcohol restrictions, MAPS review, taper, dismissal, or return-to-work provisions.
  • The revision date, footer, clinic or health-system name, insurer or Medicaid-plan name, workers’ compensation status, and document number.
  • Older versions so policy changes can be documented over time.

Protect private information

A blank form is best. On a completed copy, permanently redact the patient’s name, birth date, address, contact information, signatures, medical-record and claim numbers, employer and insurer identifiers, individualized diagnoses, prescription details, test results, barcodes or QR codes, and other private information before submission.

How Michigan submissions are classified

Each clause is compared against the Medicaid Common Formulary criteria, R 418.101008a and related workers’ compensation materials, the Start Talking requirement, and any identified clinic or payer source. A clinic-created term is not labeled “Michigan law” unless a controlling source actually requires it.

Submit

Use the national Seeds of Vice submission channel and identify the document as a Michigan Contract for Care. If known, specify Medicaid, workers’ compensation, Start Talking acknowledgment, or private clinic agreement.
