Help build Georgia’s written pain-agreement record
Georgia requires written treatment agreements in defined chronic-pain prescribing. Individual practices then add their own refill, testing, pharmacy, pill-count, substance-use, dismissal, financial, and information-sharing terms. Seeds of Vice is collecting the actual Georgia forms so those layers can be separated accurately.
What is most useful
- A blank pain-management, opioid, narcotic, or controlled-substance agreement from a Georgia practice.
- The complete packet when the agreement came with refill, drug-screen, pill-count, pharmacy, CBD/cannabis, dismissal, taper, driving, financial, privacy, or release policies.
- Any revision/effective date, footer, document number, clinic name, clinician name, and office location printed on the form.
- Older versions of the same form. Version history can show when a practice added, removed, or changed a requirement.
- Written notices showing how a practice handled an alleged agreement violation, taper, discontinuation, discharge, or transfer of care.
Why Georgia copies matter
Georgia Rule 360-3-.06 requires a written treatment agreement in defined circumstances but does not prescribe one statewide contract or a universal list of contract clauses. Actual practice forms are therefore necessary to distinguish the state requirement from clinic-created conditions.
Protect patient information
A blank form is best. On a completed copy, permanently redact the patient’s name, birth date, address, telephone/email, signatures, account/medical-record/insurance numbers, diagnoses, individualized prescriptions, test results, barcodes/QR codes, and other personal medical information before submitting it.
What Seeds of Vice does with the form
Each material clause is compared with current Georgia authority. The existence of a written agreement, monitoring, and certain clinical review duties may reflect the state rule when its scope is met. One-pharmacy rules, early-refill restrictions, lost/stolen-medication policies, pill-count deadlines, CBD/cannabis restrictions, automatic discharge language, disclosure provisions, and financial conditions are classified separately unless another binding authority independently requires them.
