Help build Delaware’s chronic-pain Treatment Agreement record

Delaware requires a signed Treatment Agreement for covered chronic-pain patients under its statewide controlled-substance regulation. Individual practices may add their own refill, pharmacy, testing, substance-use, financial, conduct, or dismissal provisions. Seeds of Vice is collecting actual Delaware forms so the regulatory minimum can be separated from practice-added terms.

What is most useful

  • A blank chronic-pain, opioid, controlled-substance, or Treatment Agreement used by a Delaware practice.
  • The complete packet if the agreement came with refill, pill-count, drug-screen, pharmacy, alcohol/cannabis, taper, dismissal, privacy, conduct, financial, or appointment policies.
  • The revision date, footer, clinic name, prescriber name, office location, and document number.
  • Older versions of the same agreement so changes can be documented.
  • Written notices showing how an alleged agreement violation, abnormal drug test, taper, discontinuation, discharge, or transfer of care was handled.
  • Forms showing how different practitioner types implement the statewide Section 9 rule.

Protect patient information

A blank form is best. On a completed copy, permanently redact the patient’s name, birth date, address, contact information, signatures, account or medical-record numbers, insurance information, individualized diagnoses and prescriptions, test results, barcodes or QR codes, and other personal medical information before submission.

How the form is classified

Each clause is compared with the Uniform Controlled Substances Act Regulations and, where applicable, the Board of Medical Licensure’s Section 18 rule. The existence of the signed agreement, six-month PMP review, six-month fluid drug screening, and risk assessment are separated from practitioner-chosen clauses involving lost medication, early refills, one-pharmacy sourcing, unauthorized substances, or agreement-violation consequences.
