Help build Colorado’s workers’ compensation opioid-contract record

Colorado’s identified Contract-for-Care requirement applies in the workers’ compensation chronic-pain framework, where covered long-term opioid management uses an opioid contract/shared-decision agreement. Seeds of Vice is collecting actual Colorado workers’ compensation and practice agreements so the program rule can be separated from private-clinic policy.

What is most useful

  • A blank opioid contract, shared-decision agreement, or chronic-pain agreement used in a Colorado workers’ compensation case.
  • The complete packet if the agreement came with drug-testing, pill-count, refill, pharmacy, cannabis/alcohol, taper, return-to-work, dismissal, or functional-goal policies.
  • The revision date, footer, clinic name, authorized treating provider, workers’ compensation carrier, and document number.
  • Older versions of the same agreement so guideline changes can be tracked.
  • Written notices showing how an alleged agreement violation, abnormal test, taper, discontinuation, denial of treatment, or referral was handled.
  • Private Colorado pain-practice agreements, clearly identified as non-workers’ compensation when applicable, so local policy can be distinguished from state program requirements.

Protect patient and claim information

A blank form is best. On a completed copy, permanently redact the patient’s name, birth date, address, contact information, signatures, claim number, employer, insurer identifiers, medical-record numbers, individualized diagnoses and prescriptions, test results, barcodes or QR codes, and other personal or claim-specific information before submission.

How the document is classified

Each clause is compared with the DWC Chronic Pain Disorder Medical Treatment Guideline, workers’ compensation opioid-management rules, statewide PDMP law, and Colorado’s general chronic-pain statute. Workers’ compensation requirements are separated from private clinic terms and from statewide duties that apply outside workers’ compensation.
