Evidence status: CURRENT AGREEMENT LISTING / FULL-TEXT GAP. Nona Spine and Pain Institute’s live Patient Forms page currently links an Informed Consent & Controlled Substance Agreement. The public PDF exists, but its complete text was not reliably retrievable during this evidence pass.

What is verified

  • The practice is currently active.
  • The live patient-forms page currently lists and links the agreement.
  • Public practice/provider materials associate Bharat C. Patel, MD with Nona Spine and Pain Institute.

What is not yet verified

The complete current agreement language. Seeds of Vice will not infer testing, refill, pharmacy, marijuana, disclosure, taper, or dismissal terms from other Florida contracts or from the fact that Florida requires an agreement.

Florida law that still applies to the classification

When treatment is within §456.44’s chronic-nonmalignant-pain scope, the written agreement must address prescription/refill number and frequency, patient compliance/discontinuation, and single-treating-registrant control. Those state minimums do not reveal what additional language Nona uses.

Next document needed

A clean current copy of the Nona Spine and Pain Institute Informed Consent & Controlled Substance Agreement.
