Evidence status: CURRENT STATE IMPLEMENTATION TOOL. New Hampshire’s Office of Professional Licensure and Certification publishes a prescriber checklist that operationalizes the chronic-pain opioid rules into a practical charting tool.

What the checklist tracks

The checklist identifies chronic-pain requirements including:

  • Written Treatment Agreement.
  • Consideration of pain consultation for patients receiving 100 mg morphine equivalent daily dose for more than 90 days.
  • Reevaluation of the Treatment Plan and opioid use at least twice per year.
  • PDMP review at least twice per year.
  • Urine drug screening at least annually for patients taking opioids longer than 90 days.

Checklist exemptions

The checklist repeats the rule’s exemption from the treatment-agreement and periodic-testing requirements for long-term non-rehabilitative facility residents whose opioids are administered by licensed staff, and for patients with episodic/intermittent pain receiving no more than 50 dose units in a three-month period.

What the checklist is

This is an implementation and compliance tool, not a substitute for RSA 318-B:41 or Med 502. It is useful because it shows how the state translates the legal standards into day-to-day prescribing documentation.
