What the packet contains
The packet includes a Prescription Medication Agreement for Controlled Substance Treatment of Pain that closely tracks Nevada’s statutory requirements and then adds practice-specific conditions.
Terms reflecting Nevada law
- Treatment goals and discussion of alternatives.
- Consent to urine, hair, blood, and other drug-use monitoring when the practitioner deems it appropriate or necessary.
- Taking medication only as prescribed.
- No sharing medication.
- Disclosure of other controlled substances, alcohol, marijuana/cannabinoid use, side effects, overdose history, and states where controlled-substance prescriptions were filled.
- Risk assessment and additional evaluation after treatment exceeds 90 days.
Practice-specific additions
- Prescriptions are provided only during scheduled office visits, with appointments typically arranged 30 days in advance for refills.
- Lost or stolen medication may not be replaced until the next appointment, at the prescriber’s judgment.
- The patient agrees to use only one pharmacy unless medication availability or cost justifies another pharmacy and the practice is notified.
- The form authorizes cooperation with law enforcement, the State Board of Pharmacy, and occupational licensing boards.
- Agreement violations may lead to denial of controlled-substance prescriptions and discharge from the clinic.
State law versus clinic policy
