Help build West Virginia’s narcotics-contract record

West Virginia law writes minimum terms into a narcotics contract, but individual practices can add their own rules. Seeds of Vice is collecting the actual forms so West Virginians can see which terms come from the state and which come from a clinic, hospital, payer, or workers’ compensation program.

What is most useful

  • A blank Narcotics Contract or clean patient-provider agreement from a West Virginia clinic, hospital, primary-care office, pain clinic, or specialist.
  • The complete packet if the contract came with refill, urine-testing, pill-count, dismissal, cannabis, pharmacy, payment, or code-of-conduct policies.
  • Any page showing a revision/effective date, institution name, provider name, address, or footer.
  • Older versions. West Virginia’s contract law has changed over time, including the timing of the trigger and the contract’s minimum terms.
  • A current copy of the West Virginia Hospital Association Addendum A Narcotics Contract.
  • A current Jackson General Hospital/WVU Medicine Narcotics Contract form.
  • Workers’ compensation opioid treatment agreements used in actual claims, with all worker-identifying information removed.

Protect the patient

A blank form is best. If the copy was completed, permanently redact the patient’s name, birth date, address, phone/email, signatures, medical-record/account/insurance numbers, diagnoses, prescription details that identify the patient, test results, barcodes, QR codes, and other personal medical information before sending it.

What Seeds of Vice does with it

The document is reviewed for source, date/version, institution, contract terms, and comparison with current West Virginia authority. Every material term is classified separately. A state-required one-pharmacy term is labeled as state law; a clinic-added drug-test deadline or refill rule is not attributed to the state unless another binding source requires it.
