Help build Vermont’s Controlled Substance Treatment Agreement record

Vermont requires a signed Controlled Substance Treatment Agreement for covered chronic-pain opioid treatment. Individual practices may add their own refill, testing, pill-count, pharmacy, appointment, conduct, financial, substance-use, or dismissal provisions. Seeds of Vice is collecting the actual Vermont forms so those layers can be separated accurately.

What is most useful

  • A blank Controlled Substance Treatment Agreement or chronic-pain opioid agreement from a Vermont practice.
  • The complete packet if it came with refill, urine-testing, pill-count, pharmacy, cannabis/substance, taper, dismissal, financial, privacy, or appointment policies.
  • The revision date, footer, clinic name, prescriber name, office location, and document number.
  • Older versions of the same agreement.
  • Written notices showing how an alleged agreement violation, abnormal test, taper, discontinuation, discharge, or transfer of care was handled.
  • Workers’ compensation notices where alleged noncompliance with Vermont’s opioid rule affected payment for chronic-pain treatment.

Protect patient information

A blank form is best. On a completed copy, permanently redact the patient’s name, birth date, address, contact information, signatures, account or medical-record numbers, insurance or claim identifiers, individualized diagnoses and prescriptions, test results, barcodes or QR codes, and other personal medical information before submission.

How the agreement is classified

Each term is compared with the 2024 Department of Health rule. State-required subjects such as functional goals, pharmacy choice, and safe storage/disposal are identified separately from prescriber-added conditions such as exact testing frequency, pill-count deadlines, refill restrictions, conduct rules, or automatic discharge.
