Help build New Mexico’s written pain-agreement record

New Mexico requires written treatment agreements for defined chronic-pain prescribing. The state rule sets a one-practitioner/one-pharmacy expectation whenever possible, while individual practices and institutions may add their own refill, testing, pill-count, substance-use, appointment, financial, disclosure, or dismissal terms. Seeds of Vice is collecting the actual New Mexico forms so those layers can be separated accurately.

What is most useful

  • A blank chronic-pain, opioid, controlled-substance, narcotic, or pain-management agreement from a New Mexico practice.
  • The complete packet if the agreement came with refill, urine-testing, pill-count, pharmacy, cannabis/substance, taper, dismissal, financial, privacy, or appointment policies.
  • The revision date, footer, document number, clinic name, prescriber name, and office location.
  • Older versions of the same agreement so changes over time can be documented.
  • Written notices showing how an alleged violation, abnormal test, taper, discontinuation, discharge, or transfer of care was handled.
  • Health-system policies or EHR instructions showing how agreement status and alleged violations are documented.

Protect patient information

A blank form is best. On a completed copy, permanently redact the patient’s name, birth date, address, contact information, signatures, account or medical-record numbers, insurance numbers, individualized diagnoses and prescriptions, test results, barcodes or QR codes, and other personal medical information before submission.

How the form is classified

Each clause is compared with the controlling New Mexico professional rule. The written-agreement mandate and one-practitioner/one-pharmacy requirement are identified as state requirements where applicable. Clinic-specific refill rules, testing frequency beyond the regulatory minimum, pill-count deadlines, substance restrictions, financial conditions, and automatic-discharge provisions are classified separately unless another binding authority independently requires them.
