Help build New Jersey’s pain-management-agreement record

New Jersey requires a pain management agreement before covered ongoing chronic-pain treatment begins. The state publishes a model agreement, but individual practices and health systems may add their own refill, testing, pharmacy, appointment, substance-use, financial, disclosure, or dismissal terms. Seeds of Vice is collecting the actual New Jersey forms so those layers can be separated accurately.

What is most useful

  • A blank pain-management, opioid, narcotic, controlled-substance, or patient-provider agreement from a New Jersey practice.
  • The complete packet if the agreement came with refill, drug-screen, pill-count, pharmacy, cannabis/substance, taper, dismissal, financial, privacy, or appointment policies.
  • Any revision date, footer, document number, clinic name, prescriber name, and office location.
  • Older versions of the same agreement so changes can be documented.
  • Written notices explaining how an alleged agreement violation, abnormal test, taper, discontinuation, discharge, or transfer of care was handled.
  • Institutional policies or EHR instructions showing how a health system implements New Jersey’s agreement requirement.

Protect patient information

A blank form is best. On a completed copy, permanently redact the patient’s name, birth date, address, contact information, signatures, account or medical-record numbers, insurance numbers, individualized diagnoses and prescriptions, test results, barcodes or QR codes, and other personal medical information before submission.

How the form is classified

Each material clause is compared with the New Jersey statute, governing professional rule, Board FAQ, and state model. Required agreement subjects are identified as such. Clinic-specific refill restrictions, pharmacy rules, test frequency, pill-count deadlines, substance prohibitions, appointment penalties, financial terms, annual renewals, and automatic-discharge provisions are classified separately unless another binding authority independently requires them.
