Help build Massachusetts’ pain-management treatment-agreement record
Massachusetts requires a written pain-management treatment agreement in defined long-term treatment involving extended-release or long-acting opioids, while professional and workers’ compensation rules may use agreements in additional circumstances. Seeds of Vice is collecting actual Massachusetts forms and policies so those layers can be separated accurately.
What is most useful
- A blank pain-management, opioid, controlled-substance, or treatment agreement from a Massachusetts practice.
- The complete packet if the agreement came with drug-testing, pill-count, pharmacy, refill, cannabis/substance, taper, dismissal, privacy, financial, or appointment policies.
- The revision date, footer, clinic name, prescriber name, office location, and document number.
- Older versions of the same document so changes can be tracked.
- Written notices showing how an alleged agreement violation, abnormal test, taper, discontinuation, discharge, or transfer of care was handled.
- Workers’ compensation agreement forms or insurer requirements that are presented as conditions of long-term opioid treatment.
Protect patient information
A blank form is best. On a completed copy, permanently redact the patient’s name, birth date, address, contact information, signatures, account or medical-record numbers, insurance or claim identifiers, individualized diagnoses and prescriptions, test results, barcodes or QR codes, and other personal medical information before submission.
How the form is classified
Each clause is compared with M.G.L. c.94C §18A, applicable professional regulations, MassPAT law, workers’ compensation guidelines, and current Massachusetts guidance. Statutory requirements are separated from practice-added conditions such as one-pharmacy rules, fixed testing schedules, pill-count deadlines, lost-medication rules, conduct clauses, or automatic discharge.
