Help build Florida’s controlled-substance agreement record

Florida requires written controlled-substance agreements in defined chronic-nonmalignant-pain treatment, but individual practices add very different terms. Seeds of Vice is collecting the actual Florida forms so patients can tell what the state requires and what a clinic added.

What is most useful

  • A blank controlled-substance, narcotic, opioid, or pain-management agreement from a Florida practice.
  • The complete packet if the agreement came with testing, refill, pill-count, pharmacy, marijuana/substance, physical-therapy, dismissal, financial, or privacy policies.
  • Any revision/effective date, footer, clinic name, provider name, or address printed on the form.
  • Older versions if you have them. Version history can show when a practice changed its requirements.
  • Current copies of the Nona Spine, Arthritis & Rheumatology Center of South Florida, Key West Orthopedics, or Internal Medicine & Pediatrics of Tampa Bay agreements, where current public listings exist but complete text remains a gap.

Protect patient information

A blank form is best. On a completed copy, permanently redact the patient’s name, birth date, address, telephone/email, signatures, account/medical-record/insurance numbers, diagnoses, individualized prescriptions, test results, barcodes/QR codes, and other personal medical information before sending it.

What Seeds of Vice does with the form

Each material clause is compared with current Florida authority. Refill frequency, compliance/discontinuation, and one-treating-registrant language may reflect Florida’s statutory minimum. Drug-testing schedules, one-pharmacy rules, cannabis restrictions, required therapy, financial conditions, information releases, and discharge terms are classified separately.
